Medical services are available to all MAAL students free of charge. However, students must have a signed parent consent form on file to receive clinic services.
Please download medical consent form below, complete information and bring completed form to the clinic on the 10th floor. English and Spanish versions of the form are available.
All students are eligible for a free flue shot with a signed consent form.
